Plague Doctor Variations:
Since there are a few Plague Doctor builds that all play fairly similar to this one I thought instead of making other guides I'll just list them here with short discriptions.
Sadhu - Plague Doctor:

Extremely similar play style to this build, Sadhu's transmit Prana is a very powerful buff that adds bonus damage to all your attacks, also gives half your Stats to allies for the duration (you don't lose stats). Possession is also a channeled aoe root attack with a short cooldown.

A more stationary playstyle, powerful in Earth Towers defense floors or Uphill defense. Also has strong utility with its other carve statues by lowering cooldowns, mana costs, giving temporary invulnerability and silencing enemies in an aoe.

Plague Doctor specializes in dealing magic damage over time with Incinerate and Black Death Steam, and spreading debuffs with Pandemic.
Bokor synergizes very well with Plague Doctor because you can spread Hexing to multiple enemies with Pandemic which causes Effigy to become an aoe that deals significant damage while your Plague Doctor abilities are dealing damage over time. Damballa is also a strong spell that causes your zombies to explode for a large burst of damage in an aoe.
We take Exorcist at Rank 10 in all 3 variations for Rubric which is a channeled aoe spell that deals holy damage over time. Even without the Circle 2 attribute its still a very strong ability.
Rating of Class 1-10 (my opinion):
Single Target: 7
Area of Effect: 8
Durability: 7
Solo: 7
Team: 7
Pros:
- Plays like a Wizard while being a Cleric
- Low cooldown skills
- Great Aoe
- Can deal with flying monsters easy
- Flexible Rank 6.
Cons:
- Sp hungry (solved by potions/gear with spirit)
- No Resurrection
Other:
- No real strengths or weaknesses, all around fairly balanced.
Stat Distribution:
0-70 CON, Rest INT, See below for Spirit.
70 invested Con will give you a good amount of base Hp, However you may require higher health for some of the more dangerous content in the game.
As for Spirit, you will want to increase your Sp pool since ability costs are medium to high compared to some other builds. I suggest avoid spending your attribute points on Spirit and instead getting it through gear, you can easily get some Int + Spr purple gear for fairly cheap until you upgrade to something better like Primus with the same stats, Velcoffer or crafted.
However, you can invest around 100 spirit in your stats if you wish and stat reset later when you have significantly better gear.
Skill Distribution:
You will notice that some circles don't have 15 points invested, that's because you can save points from the same class and invest them in the next circle.
Cleric:
10/10 Heal, literally Clerics best spell in the game, every cleric maxes this.
10/10 Cure, Scales very well with Int builds and even on non int builds, also dispels debuffs if you walk on it and counts as a debuff on monsters for Incinerate.
4/10 Safety Zone, Four points is enough thanks to the +20 Block Attribute.
5/5 Divine Might, A great skill because it causes your next x amount of skills (x depends on Divine Mights level) to be +1 level higher.
1/5 Fade, One point is enough for removing aggro so you can interact with objects or avoid some ground spells.
Bokor:
14/15 Hexing, One of the builds most important skills, allows you to use Effigy and lowers magic resist monsters.
15/15 Effigy, Deals a single hit of magic damage to all enemies affected by Curse and every third hit deals a multiplied amount of damage.
You can use Pandemic (Plague Doctor skill) to spread Hexing which will cause Effigy to hit all the targets affected by the curse.
4/15 Zombify, Zombies won't be dealing a ton of damage since they scale with SPR but you can suicide them with Damballa for great damage, also now summons scale with your weapon they can still deal fair damage while you are casting spells yourself. 4 points for 100% up time.
1/10 Mackangdal, Utility spell I rarely find the chance to use, but worth investing one point into since you aren't sacrificing anything to get it really. You may skip this if you wish though.
1/10 Bwa Kayiman, Causes your Zombies to follow you like a train dealing damage to what they come in contact with.
5/5 Ogouveve, Increases your Zombies damage significantly. You also level this skill for the 'Ogouveve: Decreased Strength' Attribute for an extra debuff to increase the duration of Plague Doctors Incinerate.
5/5 Damballa, Causes all your Zombies to explode dealing damage in an aoe, leveling an attribute gives them a chance to respawn when they explode.
Dievderby:
5/5 Laima Statue, Take a moment to carve a statue and once finished causes anyone nearby to receive its benefits of 20% cooldown reduction on their abilities. Enemies nearby are slowed and is another debuff for your Plague Doctors Incinerate. Lasts 55 seconds at level 6.
5/5 Zemyna Statue, Take a moment to carve a statue and once finished causes anyone nearby to receive its benefits of reduced Sp costs on abilities. Lasts 55 seconds at level 6.
5/5 Vakarine Statue, Take a moment to carve a statue and once finished you may interact with it which allows you to teleport as if it were a normal Vakarine teleport statue. 5 people may use it at level 5 and lasts 30 seconds.
Plague Doctor:
15/15 Incineration, Hits enemies in a small aoe in front of you. Only works on monsters that have at least one debuff on them, deals fire damage over time for 10 seconds and adds 1 second per debuff on it when cast. This skill is enhanced by Agny Necklace.
Debuffs this build utilizes:
- Cure
- Hexing
- Zombify
- Ogouveve
- Laima Statue
- Black Death Steam
11/15 Pandemic, Spreads certain debuffs to other monsters in a small radius (large if you level the attribute).
This allows you to spread Hexing so Effigy becomes an aoe, Black Death Steam so it hits even more monsters and Incineration with an attribute leveled.
10/10 Black Death Steam, Hits enemies in a medium aoe infront of you. Deals poison damage over time and counts as a debuff for incineration, also can be spread with Pandemic.
7/10 Healing Factor, Lays a magic circle on the ground that when picked up snapshots whatever hp you were at and will continuously regenerate health until you reach that snapshot hp.
For example if you are at 50% hp then pick it up and then take damage you will regenerate hp at a steady rate until you reach 50%, but not past 50%. So this skill is more useful at high health.
1/5 Bloodletting, Useful to use against certain enemies, like the boss that sleeps you in Canyon Mission or Elite monsters in the world.
1/5 Methadone, Buffs the entire party which makes you immune to knockback, knockdown and casting interruption, but increases your damage taken by 20%. Its quite a powerful buff since its basically Pain Barrier for the party but with the cost of taking 20% more damage, be careful when and where you use this, but can be extremely useful for yourself and others.
Exorcist:
5/5 Rubric, Channel spell that hits for a ridiculous amount of holy damage in a long straight line. Even without its attribute you learn at Circle 2 its still very powerful.
5/5 Aqua Benedicta, Throw a bottle of holy water at target location that deals holy damage over time for several seconds. 25 second cooldown with 3 overheat is what makes this spell quite good, unfortunately the downside is that it cannot hit flying enemies.
5/5 Engkrateia, Since Entity basically does nothing for us until Exorcist C2, we put our remaining points into this. It only lasts 3 seconds, but its basically a more powerful Pain Barrier that makes you take less damage. Situational and hard to use, but good.

Equipment / Core Items / Cards:
Use a Rod & Shield. Cloth for more Mdef, Plate for more Pdef.
(Optional) Agny Necklace, Adds 50% skill factor to your Fire skills (Incineration).
Aka it turns Incineration from 173% to 223%.
Incineration may not be the a huge percent of your overall damage but it is still significant, if you don't mind sacrificing some stats you may gain from a higher level more expensive necklace, this is an option.
Red Card: Chapparition Lv10 x3 (+30% Magic Attack for 6 seconds after consuming an Sp Potion)
Blue Card: Nuaele Lv10 x3 (+30% Magic Defense)
Green Card: Pyroego Lv10 x3 (+30 Intelligence)
Purple Card: Gazing Golem Lv5-10 x3 (15-30% chance to gain Pain Barrier when hit)

Also, I would take 1 or 2 points from Cleric's Safety Zone into Deprotected Zone, which is 6~8s duration plus 5s from its attribute, for another debuff on our list (the % defense reduction is negligible but at least it has a 50 raw defense reduction, which does not take stacks into account).
deprotect zone is kind of bad, wish they'd rework that skill, i wouldnt even invest one point for the debuff either, its up to you though, it wouldnt change much losing 1 point really.
Also PD3 isn't really much of a support class since it doesn't really have much support skills that are useful in pve, its more of a damage class.
Krivis C2 (or any of krivis's circles) doesn't really offer much to this build at all though.
Miko quest isn't that bad tbh, its pretty easy now that there isn't any competition for the spirits, BUT yeah you'd have to sex change then change back to male after you become a Miko which can be a cost not worth paying for.
Paladin really just offers access to a 2H Mace which is around +100 base matk to a Rod which can be worth going for, I personally like having a shield since you can make up for stat loss there while still having some more defense too.
If you dislike Miko you could really fit anything there if you wanted, its not a core part of the build by any means, Diev is a pretty good solo circle as well and helps with the SP costs.
You also could go the Sadhu version since you are only taking one circle of Bokor and taking Sadhu circle 3, the class is no longer a meme and actually has some good damage and utility as well.

- extra damage with Zaibas
- Aukuras is good for support
- Zalciai for sweet crits, but not taking it coz this is an Int build...
- And Last, the new Daino which makes ur normal attack become magic - really good since this is an Int build

druid cant say, never really tried it but you don't have to go miko, you could always go Diev, Priest or something else.
But anyways, no harm done! :)
You can get more sp from gear, the build does go through sp fairly fast (mostly because of Pandemic spread), but its not that bad once you get some decent gear and use potions.
If you are worried about sp you could always go the Sadhu version, it doesn't spam effigy as much and its lower level, the build is just as strong too.
